A. I don't know where you live so I'm just gonna tell you information for both - the US and the UK...
In the US i believe that you have to sign a 2 year contract in either Apple Store or AT&T unless you buy it on eBay or Amazon... So the iPhone 4 is for $199 or $299 with a 24 month contract or you can get it for like $599 on eBay without a contract.. Also all the iPhones are unlocked even when you buy it from the Apple Store...
In the UK they sell them in Apple Store for £499 or £599 without a SIMcard so that means it's unlocked and you get the SIMcard from the carrier you'd like.. All carriers sell it in the UK apart from Virgin Mobile... Even the Tesco Mobile.. If you decide to buy the iPhone from the carrier you'd get it cheaper... For example you buy the iPhone from Vodafone for £485 and no top-up is required but you get it for the same price with Orange but you have to top-up.. You can still get it for free with a 24 or 18 month contract so you have a plenty of options there.. Oh and if you live in the UK then I say you get it at Vodafone because they are the cheapest network and they won an award for being the best company to sell the iPhone.. So I hope I helped.. :))

A. Walmart, but not by much. $197/$297. In that case you could get it for the $199/$299 price at Best Buy, RadioShack, AT&T stores & Apple Stores.
